Third Party Consent Disclosure

Such information shall include, but is not limited to, employment history and income or wages; bank, money market, and similar account balances; verification of assets; credit history; copies of income tax returns; and rental/mortgage verifications. By authorizing Citywide Home Loans, LLC, I understand l also agree to the Third Party’s applicable Privacy Policy, Terms and Conditions and other similar agreements listed below (as updated from time to time by the Third Party) as a condition to Citywide Home Loans, LLC receiving information or verification from such Third Party. This authorization is valid for any party named in the preapproval or loan application. I/We understand and agree that Citywide Home Loans, LLC reserves the right to change the mortgage loan review process to a full documentation program. This may include verifying the information provided on the application directly with the employer and/or the financial institution.
